The Certified Nursing Assistant (CNA) delivers safe, reliable, and compassionate hands-on care to clients while maintaining clinical excellence and a best-in-class service experience. You’ll support activities of daily living (ADLs), monitor and document changes in condition, and collaborate tightly with nursing and interdisciplinary teams to ensure timely, dignified, and person-centered care.

Position: Certified Nursing Assistant (CNA)

Reports to: Clinical Supervisor (RN) / Director of Nursing

Essential Duties and Responsibilities:

Patient Care

  • Provide basic patient care under the supervision of an RN, including ADLs: bathing, dressing, grooming, toileting, feeding, transfers, ambulation, and positioning.
  • Monitor and record vital signs; promptly escalate changes in condition.
  • Offer companionship and emotional support to clients and families, maintaining dignity and respect at all times.

Safety and Compliance

  • Follow infection-prevention protocols (hand hygiene, PPE) and adhere to agency policies and HIPAA.
  • Utilize safe-patient-handling techniques and prescribed mobility equipment; comply with fall-prevention practices.
  • Maintain a clean, safe care environment consistent with the plan of care.

Customer Service

  • Communicate professionally, courteously, and empathetically with clients, families, and the care team.
  • Demonstrate reliability, punctuality, and service excellence during every interaction.

Documentation and Reporting

  • Document care and vitals accurately and on time in the EHR/EVV; report changes in health or behavior promptly to the supervising nurse.

Other Duties

  • Participate in in-services, competency checks, and staff meetings to stay current on policies and procedures.
  • Perform related tasks as assigned by nursing leadership consistent with the scope of practice.

Work Environment

  • Care is provided in home and/or facility settings with routine exposure to standard healthcare environments.
  • Role requires frequent travel between client locations, depending on the assignment.

Qualifications

  • Current CNA certification in the state; in good standing on the nurse aide registry.
  • Prior CNA experience in home health, assisted living, SNF, or hospital preferred (new CNAs welcome).
  • Current BLS/CPR (AHA/Red Cross) preferred or obtained upon hire.

Skills and Competencies

  • Strong understanding of basic patient care and ADLs; confident with mobility/transfer assistance.
  • Excellent communication, documentation, time management, and customer service skills.

Physical Requirements

  • Frequently lift, pivot, and transfer up to 50 lbs with/without assistive devices; stand/walk for extended periods.
  • Bend, kneel, and reach to support personal care tasks; operate lifts and mobility equipment safely.
