The Licensed Practical Nurse (LPN) provides nursing care under the supervision of a Registered Nurse (RN) in accordance with the North Carolina Board of Nursing and home care company policies. The LPN delivers direct client care in the home setting, supporting the health and well-being of clients through nursing interventions, monitoring, and education. This role requires adherence to all applicable North Carolina statutes and regulations for home health care.

Position: Licensed Practical Nurse (LPN)

Reports to: Clinical Supervisor (RN) / Director of Nursing

Location: Morganton, NC 

Essential Duties and Responsibilities:

Client Care

  • Provide direct support for client care admissions, including the development of plans of care. 
  • Assist with the development of activities of daily living (ADLs) as needed, ensuring the comfort and safety of clients. 

Monitoring and Reporting

  • Monitor clients’ conditions and report changes or concerns to the supervising RN and other members of the care team. 
  • Document care provided and client responses accurately and timely in compliance with company policies and state regulations. 

Client and Family Support

  • Educate clients and families about managing health conditions and provide guidance on proper care techniques to support client independence and safety. 
  • Offer emotional support to clients and their families, fostering a compassionate and supportive care environment. 

Collaboration and Coordination

  • Collaborate with the interdisciplinary team, including RNs, physicians, therapists, and other healthcare professionals, to ensure comprehensive and coordinated client care. 
  • Participate in care planning meetings and contribute insights on client care needs. 

Compliance and Documentation

  • Ensure compliance with all North Carolina state statutes and regulations governing LPN practice in home care. 
  • Maintain accurate and complete medical records, following the agency’s documentation guidelines and regulatory standards. 

Emergency and On-Call Duties

  • Respond promptly to client emergencies within the scope of practice and provide appropriate care until additional help arrives. 

Skills and Qualifications

  • Must hold a current, unencumbered Licensed Practical Nurse (LPN) license in the state of North Carolina. 
  • Graduate of an accredited practical nursing program. 

Experience

  • Minimum of one year of clinical experience, preferably in a home health, hospice, or long-term care setting. 
  • Familiarity with home care principles, practices, and state regulations in North Carolina. 

Skills 

  • Excellent communication and interpersonal skills to interact with clients, families, and healthcare team members. 
  • Proficiency in using electronic medical records (EMR) and other relevant healthcare technology. 

Physical Requirements

  • Ability to travel to client homes and perform tasks related to client care, including lifting and moving clients as necessary.
  • Must be physically capable of performing nursing duties in various home environments. 

Other Requirements

  • Valid driver’s license and reliable transportation. 
  • Current CPR certification. 

Compliance

  • The LPN must follow the standards of practice as defined by the North Carolina Board of Nursing and the policies of the home care company. They must provide nursing care that meets professional standards and regulatory requirements, ensuring the safety and well-being of clients in the home care setting.
